Submitting behaviour support plans and reports

How to lodge behaviour support plans

Any behaviour support plans containing regulated restrictive practices must be lodged by specialist behaviour support providers with the NDIS Commission.

Lodgement occurs in the NDIS Commission portal and requires a PRODA account.

Lodgement involves entering details about the participant, the regulated restrictive practices and any implementing providers and attaching copies of the behaviour support plan and where relevant the functional behavioural assessment.

The format of the behaviour support plan

NDIS behaviour support practitioners can use the NDIS Commission’s behaviour support plan templates, their own template or any template specifically required by the state or territory restrictive practice authorisation body.

The NDIS Commission has undertaken a review of the Interim and Comprehensive Behaviour Support Plan Templates. Practitioners and provider are encouraged to use the revised resources:

As a transitional arrangement copies of the previous templates V2.0 will remain available for use together with a summary of findings which informed the new templates (V3.0).

This template can be used as a guide to complete an interim behaviour support plan. This form is approved by the NDIS Quality and Safeguards Commissioner for the purposes of section 23 of the National Disability Insurance Scheme (Restrictive Practices and Behaviour Support) Rules 2018.
